Digital Preservation staff at the Minnesota Historical Society have been using this form since 2016 (approximately) as a way to make informed decisions about collections and how they are stored.  The form serves as a starting point for a conversation between curators and digital staff that balances content, media, format, institutional obligation to the content; ultimately each collection is assigned a service tier and resources are applied accordingly.  It also captures important planning information about restrictions, possible additions, and recovery needs.
